These forms are used by the Graduate College faculty. They often require the approval of the advisor, department chair, dean and Graduate Dean before they can become part of an approved plan of study.
Faculty who are applying for regular appointment must submit their vita in electronic format to the Graduate College office. The original application must have signatures before it can be processed.
Students requesting a substitution of a course from another university, must submit an official transcript to the Gradaute College office. A courses being transfered into a program must have a letter grade of " A" or "B."
Contract must be approved prior to student enrolling in a course.
