Transcripts: Entering freshmen must send their most recent high school transcripts Students who have already started college should send their most recent college transcript.

ACT Scores: Entering freshmen must send a copy of their ACT scores. Students who have already started college may send their ACT (or SAT) scores if they are reasonably available (these may already be on your college transcripts).

III. – VI. Tables (This section is optional.)

You are not required to have information for any of these tables. Include information that you think will help us understand your motivation and your capabilities.

VII. References

This section is required. Your letters for reference must be blind; you waive the right to know what your references write or say. Provide each reference with a signed copy of the reference form and a stamped, regular business-sized envelope addressed to the above address.

You should include at least two references from previous or current teachers or instructors. References from math or computer teachers will be the most useful.

A blank reference fom (PDF) is provided. You may copy or print as many copies of this form as needed. Before you give the forms to your references, clearly print your full name on them and be sure to sign the box that the reference is confidential and that you will not read it before they send it, and that you agree to let Northeastern State University contact them and discuss your qualifications.

These must be mailed separately to the above address by your references. By listing these persons (references) here, you are giving us your permission to contact them and follow up with questions about your qualifications for this scholarship.

FAFSA

Complete your application for Federal Financial Aid (FASFA) online and your follow-up information with NSU as soon as possible. This process will include sending copies of completed federal tax returns for students and parents. You should start this process as early as possible. If you need assistance completing the FAFSA, contact the NSU Office of Student Financial Services at 918-444-3456.
